Contact forms

The general contact form collects your name, email address, optional phone number, subject, and message. Submissions are sent to Anova through the Polsia email service. The contact route does not save these messages in this site's PostgreSQL database. Please do not use the general form for urgent requests or include sensitive medical details.

The Anova Luxe inquiry form sends your name, email address, optional phone number, and selected question topic to Anova through the same contact route. Submitting it does not enroll you or process a membership payment.

Email signups

When you submit an email signup form, the site stores the name, email address, and source page in its PostgreSQL database. This site does not provide a self-service unsubscribe control. Contact Anova to ask how the list is used and how to request a change.

Website analytics

The site's analytics records can include the page route, event type, referrer, and browser user agent. If the Polsia page beacon is enabled, the site also stores a randomly generated visitor identifier in browser local storage and sends the page identifier and visitor identifier to Polsia's analytics service.

Review request links

Personalized review request links may include your name, email address, and a request ID in the URL. When you choose a review platform, the website receives the platform choice, request ID, and email field; when a valid request ID is present, the site records which platform was selected and then redirects you to that platform. Reviews are submitted to the outside platform and are subject to its privacy practices. Avoid sharing a personalized review link with others.

Photo submissions

Photo submissions may include your name, email address, treatment type, optional treatment date and caption, and before-and-after photos. Photos are uploaded through the Polsia R2 service. The site stores submission details and the public CDN URLs returned by that service, and includes the URLs in an email to Anova staff. Anyone with an exact URL may be able to view a photo, including when the photo is not listed in the website gallery. Gallery publication is optional and requires separate permission and staff approval. Removing a photo from the gallery does not remove or disable its direct URL.

Do not upload a photo unless you understand this URL limitation. Contact Anova with questions before submitting.

Other services

Booking links open PatientNow. Email, analytics, and photo uploads use Polsia services. Those services process information under their own terms and privacy practices. This notice describes observed website flows; it does not establish Anova's HIPAA status or describe information collected directly by an outside service.
